- 3 Photocopies of Certified Marriage Certificate (Local or PSA Copy).
- 4 Copies of Joint Affidavit of Legitimation by Subsequent Marriage (Get from Legal Office, 2nd Floor).
Affidavits (Get from Legal Office, 2nd Floor)

- 4 Copies of Affidavit of Admission of Paternity (if applicable).
- 4 Copies of Affidavit to Use the Surname of the Father (AUSF).
- Note on signing AUSF: 0-6 yrs old (Mother signs), 7-17 yrs old (Child signs, mother attests), 18+ yrs old (Child signs).
