Step-by-Step Process
Follow these steps to complete this service.
Submit Forms
Present the duly accomplished birth certificate form and complete requirements to the LCR Office for review.
Pay Fees
Once checked, you will be issued an Order of Payment. Proceed to the Municipal Treasurer's Office (Window 2 or 3) to pay the corresponding fees.
Registration & Signature
Present your Official Receipt back to the LCR Office. The staff will register the certificate and it will be signed by the Municipal Civil Registrar.
Claim Birth Certificate
Final StepReceive your officially registered Birth Certificate. (Note: Late registrations have a mandatory 10-day waiting period before release).

Standard Requirements (Timely & Late)
5 items to prepare
- Duly accomplished birth certificate form (4 original copies).
- National ID or EPhil ID. (Required for 1 year old and above. If none, register first with PhilSys).
- 2x2 Picture with white background.
- Two (2) documents each from the father and mother proving identity (e.g., PhilHealth MDR, Gov't ID, Voter's Record, Marriage Contract, etc.).
- Barangay Certification or Proof of Residency (1 original copy).
Additional for Late Registration
2 items to prepare
- New PSA Negative Certification of Birth (Required for 7 months old and above).
- One (1) document showing date and place of birth (Immunization Record, Baptismal, Marriage Certificate, PhilHealth MDR, SSS Record, NBI/Police Clearance, or Passport).
For Unmarried Parents
1 item to prepare
- AUSF (Affidavit to Use the Surname of the Father) if the child will carry the father's last name.
